Our nurses work alongside your physiotherapist and occupational therapist to keep progress going between their visits — after a stroke, surgery, fall, or serious illness.

Recovery happens between visits

Most recovery happens between the once-or-twice-weekly visits from a therapist. Families need a nurse to keep it on track.

Stroke

Mobility re-training, safe transfers, monitoring for changes.

Surgery

Post-operative mobilisation, wound care during recovery, pain observation.

Falls & fractures

Supervised movement, fall prevention, gradual return to walking.

Hospital deconditioning

Building strength and routine after an extended hospital stay.

Long-term conditions

Parkinson's, advanced dementia, late-stage chronic disease.

What our nurses do

Supporting the rehab plan

Our nurses are not physiotherapists. They are SNB-registered nurses who support the plan set by your therapist or doctor.

Mobility and transfers

  • Safe transfers between bed, chair, wheelchair, and bathroom
  • Walking support with frames, sticks, and aids
  • Fall prevention — home setup advice, identifying risk

Supervised exercises

  • Carrying out the exercises your physiotherapist has prescribed
  • Encouraging consistency, recording progress, reporting back
  • Adjusting effort if your loved one is fatigued or in pain (within the plan)

Observation and reporting

  • Daily monitoring of vital signs, mood, pain, appetite
  • Watching for setbacks — new weakness, swelling, breathlessness, infection
  • Communicating with your therapist, doctor, and family between formal visits

Family training

  • Showing family members how to assist with transfers and exercises safely
  • Teaching what to watch for, and when to call for help
  • Building confidence so care can continue when our nurse is not there

Working with your therapist

We follow their plan, and report back

Most families already have a physiotherapist or OT — assigned at discharge, referred by a doctor, or sourced privately. Our nurses follow their plan and report back to them.

If you don’t yet have one, we can refer you to {{Partner Clinic Legal Name}}, HCSA Licence No. {{Licence No.}}, or to other registered providers depending on your location and needs.

What's outside our scope
We provide nursing support during recovery. We do not perform physiotherapy or occupational therapy ourselves — those are regulated practices delivered by registered allied health professionals. If your loved one needs hands-on physiotherapy or OT, your therapist provides it. We support the plan in between.
